Clean Air for Schools - Application form

Thank you for applying to receive free HEPA portable air cleaners from the Colorado Department of Public Health and Environment’s (CDPHE) Clean Air for Schools program! This online form must be filled out by a school administrator or other school personnel. If you are a teacher, nurse, parent, student, or other stakeholder interested in the program, share this form with your school’s administrators.
